In 2005, CRHS created an off-shoot of the U.S. Department of Health best-practice program, the Diabetes Heath Disparities Collaborative.

To supplement primary care, patients meet individually with a nurse practitioner to utilize health education resources for better daily management of their condition.

Services include:

     • Planning of self-management goals
     • Clarification of medication schedules
     • Instruction on using a glucometer to test blood sugar level
     • Importance of daily exercise
     • Explanation of proper eating habits using the Plate Method Plan:
               -Portion control
               -Shorter intervals between meals
               -More vegetables
               -Fewer carbohydrates
