CRHS patients are assigned to a medical team consisting of a medical provider and Medical Assistants
who ensure that they receive comprehensive care, including:
• Primary and preventive health care
• Management of chronic health conditions
• Health education and case management services
• School physicals and developmental assessments
• Screening and treatment for anemia, lead poisoning, and sexually transmitted diseases for CRHS patients
• Treatment of acute illness and injury
• Referrals for specialty care and low- or no-cost hospital admissions
for the uninsured
• Physician on-call when the clinic is closed
• Assistance obtaining free or low-cost medications
• Subsidized laboratory work
• Free immunizations through the District’s Vaccines for Children Project
